UPVI Data Privacy Notice

The UP Vanguard Inc. (UPVI) is well-aware of the need to keep the personal data of its stakeholders (e.g., members, applicants, and employees) private and secure, and to ensure that the same is used solely for legitimate purposes.  

UPVI’s goal is to comply with Republic Act No. 10173, otherwise known as the “Data Privacy Act of 2012” (DPA), as well as the issuances of the National Privacy Commission (NPC).   We are cognizant that personal data comprises one’s identity and that the security of personal data is of utmost importance. In this respect, kindly go through this Privacy Notice, which explains how we process the personal data we request from you.

The term “personal data,” refers to either personal information, sensitive personal information and/or privileged information.  The first two (2) are commonly used to identify you. You may refer to the DPA for more information.

We collect the following personal information from our members and applicants:

We collect the personal information enumerated above for the following purposes:

  1. Determine the health status of UPVI members given the COVID-19 pandemic
  2. Extend help to UPVI members and applicants against COVID-19 and its effects

How We Collect and Process Your Personal Information

Collection

Data will be collected using a self-accomplished electronic questionnaire via Google Forms.

Storage and Access

Your personal data will be stored in an encrypted computer database, and will only be accessed by our authorized personnel and contractor/s who hold such information under strict confidentiality. Your personal data will not be shared to third persons unless you consent, or it is required or permitted by law.

Security

Reasonable organizational, physical, and technical security measures shall be implemented for the protection of personal data which we collected. These steps of protection shall aim to maintain the availability, integrity, and confidentiality of personal data against accidental or unlawful destruction, alteration, and disclosure as well as unlawful processing.

Any security breach with regard to said personal data which comes to our knowledge will immediately be acted upon.  UPVI will take the necessary steps to address said breach and mitigate its negative effects, pursuant to the Data Privacy Act.

Recipients and Confidentiality

Only UPVI members and personnel, as well as authorized contractors shall be given access to your personal data. UPVI shall operate and hold your personal data under strict confidentiality.

Data Retention Period

We shall retain personal data for as long as necessary to accomplish the purposes stated above (i.e., for the duration of the COVID-19 pandemic), unless the law requires a longer retention period (such as when necessitated by an ongoing official investigation by law enforcement authorities). Your personal information shall be disposed of in a manner compliant with the requirements of the DPA.

Rights of Data Subjects

As a data subject, you have rights under the DPA, which include the right to access, correction, as well as the right to lodge a complaint before the NPC should there be a violation of your rights under the DPA.
